Nexium vs Prilosec

There are a lot of inaccurate information about Nexium vs Prilosec on the Internet, so we shall understand more.

Prilosec and Nexium are proton pump inhibitors (PPIs). Synonym - proton pump inhibitors. Are drugs, which inhibit the secretion of hydrochloric acid (HCl) in the stomach, so they are used and antisecretory agents for treatment of gastric hyperacidity. Proton pump inhibitors (proton pump blockers) reduce the secretion of hydrogen ions (H +, or proton) parietal (parietal) gastric cells. Secretion mechanism is in receipt of extracellular potassium ion (K +) in the cell in exchange for elimination of a hydrogen ion outwardly (H +).

Classification and Characterization

Currently there are 3 groups of drugs that reduce the acidity in the stomach:

proton pump inhibitors - are the most powerful antisecretory agents reducing formation of hydrochloric acid in the stomach. Accepted 1-2 times a day;

The H2-blockers (pronounced "al-two") - have low antisecretory efficiency and therefore can be administered only in mild cases. Taken 2 times a day. Histamine (H2-) receptors of parietal cells of the gastric mucosa. For H2-blockers are ranitidine and famotidine.

For reference: H1-blockers are used against allergies (loratadine, diphenhydramine, cetirizine, etc.).

Antacids (translated as "against acid") - means based on magnesium or aluminum compounds that are rapidly neutralized (tie), hydrochloric acid in the stomach. These include Almagelum, Aluminium phosphate gel, Maalox, and others acts quickly, but the short-term (within 1 hour), so they often have to make -. 1,5-2 hours after meals and at bedtime. Although antacids reduce gastric acidity, but also increase the secretion of hydrochloric acid by a negative feedback mechanism since tries to return the body pH (acidity level is from 0 to 14; below 7 - acidic environment above 7 - alkaline exactly 7 - neutral) to the previous values (normal pH in the stomach is 1.5-2).

Price comparison

Prilosec is several times cheaper than Nexium.

Generics (analogues) of Nexium are much cheaper

Differences between Prilosec and Nexium

Nexium is S-stereoisomer of Prilosec (levorotatory optical isomer of Prilosec), which is different from the dextrorotatory isomer as different left and right hand or the left and right shoe. It was found that R-form of Prilosec is much stronger (the S-form) is destroyed when passing through the liver, and therefore does not reach the gastric parietal cells. Prilosec is a mixture of these two stereoisomers.

According to the literature, Nexium has considerable advantages as compared with Prilosec, but is more expensive. Nexium taken in the same dosage as that of Prilosec.

Advantages of Nexium compared to other PPIs

Nexium effect starts within 1 hour after administration and lasts 24 hours. The drug acts in a wide range of pH (0,8-4,9).

Nexium is dosage is 2 times lower in comparison with Prilosec that gives better tolerability and fewer side effects. For example, in one study of side effects (headache, dizziness, diarrhea, nausea, skin rash) occurred in 2% of the treatment Nexium and 15% Prilosec.

Nexium reciept into blood from the gut (bioavailability) is independent of mealtimes.

Nexium reliably inhibits the secretion of hydrochloric acid, because its destruction in the liver does not depend on the genetic diversity of variants of the enzyme cytochrome P450. This makes it possible to better predict the effect of the drug in different patients. Nexium less than other drugs affect the metabolism (breakdown) of other drugs.

After discontinuation of Nexium is there is no syndrome of "rebound" (cancel), i.e. there is no compensatory sharp increase in the level of acidity in the stomach. The secretion of hydrochloric acid is recovered slowly (within 5-7 days).

Indications for receiving proton pump inhibitors

  • Gastric ulcer and 12 duodenal ulcer,
  • gastroesophageal reflux disease (throw acidic stomach contents into the esophagus)
  • pathological hypersecretion of hydrochloric acid (including Zollinger-Ellison syndrome)
  • in the combined treatment is used for eradication (elimination) of Helicobacter infection pylori (Helicobacter pylori) that causes ulcers and chronic gastritis.

Note. All proton pump inhibitors are destroyed in an acid medium so produced in the form of enteric capsules or tablets, which are swallowed whole (can not chew).

Conclusions

Details: Nexium has a number of advantages over other proton pump inhibitors and performance comparable only with Prilosec, Nexium but treatment is 5 times more expensive than Prilosec and slightly more expensive compared to Prilosec.

According to the literature, the efficiency of eradication of Helicobacter pylori does not depend on a specific inhibitor of proton pump (can be any), whereas in the treatment of gastro esophageal reflux disease most authors recommend to take Nexium.
